I further consent to allow the laboratory testing service to make the results of such screen available to the prospective or current employer, Jomar Contractors.
In consideration for such services being rendered on my behalf, I hereby release the laboratory testing service, its officers, agents, and employees, from any and all claims which I might otherwise have due to such results being made so available. I hereby consent not to file any action at law or in equity against Jomar Contractors, the laboratory testing service, their respective officers, agents or employees in connection with the results of such screen being made so available, and I hereby agree to indemnify and save harmless Jomar Contractors, the laboratory testing service, their respective officers, agents, and employees from all damages, expenses, reasonable attorney's fees, and costs of court which they or any of them may suffer or incur, jointly or severally, due to the results of such screen being made so available.
I authorize Jomar Contractors to investigate information concerning my education, employment experiences and all other aspects of my background relevant to my proposed employment. I release Jomar Contractors and its employees from all liability from such investigation. Jomar Contractors is an equal employment opportunity. Consumer Credit Report Disclosure and Authorization Form
A consumer report may be obtained on you for employment purposes. DRUG AND ALCOHOL TESTING CONSENT FORM
I hereby give my consent for Jomar Contractors, Inc. dba Jomar Electrical Contractors (“the Company”), or any third party or company designated by the Company in its sole discretion, to conduct any drug or alcohol tests on me that the Company deems necessary. This includes, but is not limited to, pre-employment testing, onsite testing, random testing, post-accident testing, reasonable suspicion testing, return-to-duty testing, and follow up testing.
I likewise authorize the Company or its designee to take the necessary specimens from me to test for drugs, alcohol, or other controlled substances, without advance notice, and I authorize laboratory or medical personnel retained by the Company or its designee for these tests to release the test results and related information to the Company or its designee.
I understand that any positive test results, my refusal to submit to testing as required, or any attempts to alter or falsify the results of any drug or alcohol test, may result in: (i) if I am an applicant for employment, my application for employment being considered to have been withdrawn and my being denied employment, or (ii) if I am an employee, my removal from the Company’s premises or worksites and discipline up to and including termination of employment.
